The following items are required by Davis School District and Renaissance before registering for school and we CANNOT register your student without these items:
  1.  Original Birth Certificate
  2.  Required Demographic Form – Please print, fill out and bring with you.
  3. Student's Immunization record – Immunization Requirements 2020-2021
  4. Proof of Residency in Davis School District boundaries – Examples of a document that could be used as "Proof of Residency" are: mortgage statement, water bill, gas bill, power bill, contract to build or lease agreement with your name and address on it. P.O. boxes are not acceptable. If you are using a utility bill as proof of residency, you must bring two separate bills; e.g. cable bill and a power bill. Please see the Proof of Residency Procedure form for acceptable documents.
  5.  Check-out papers from the last school attended, with the school's complete address and phone number.
  6.  If the guardian is not the student's parent, a Guardian Status form is required. – Please print, fill out and bring with you.  (Step parent must provide documentation of marriage to the student's parent.)
  7. Picture ID of parent or legal guardian.
Required Registration Fees are due when your student enrolls.
